Carlo was the son of a Jewish judge and nameless woman in Italy. During World War II, the judge had his family take refuge in Padova, in the house of Giovanni Bozzi and his mistress Esmeralda Messori. Influenced by Messori, Bozzi betrayed them and gave them up to the Nazis. They were arrested by Biagio Dezzan and taken to a concentration camp, where they eventually perished a month later. Through unspecified means, Carlo survived and traveled back to find his father. Growing up, Carlo heard his father rant about how Bozzi and Messori needed to pay and wanted to see them brought to justice. This influenced Carlo's future actions.

In 1976, Carlo's father found Bozzi, Messori, and Dezzan. He put them on the jury for a murder trial involving a man named Pasquale Ferrante, wanting to remind them they had essentially murdered his family. After serving two months in prison, Ferrante escaped. Carlo used this time to take revenge. He snuck into a pharmacy where Dezzan worked and slit his throat. As he was making his escape, he was interrupted by an actress named Mara, who stopped by to get some aspirin. Carlo blocked the door and told her that they were closed. After she had returned to her taxi, Carlo fled out the door. Dezzan's body was discovered by his wife.

Fearing that Mara would be able to recognize his voice, as he works as her script writer, Carlo adds Mara to his list of targets. That night, he tries to go after Mara, but the neighbor's dog interrupts him and he runs away. After the attempt on her life, Mara goes to live with her boyfriend Lukas Carmen, who becomes determined to help find the killer before he can kill Mara.

The next night, Carlo breaks into Messori's house and forces her face into a frying pan in the oven, burning her and finishing her off with a fork.

The next day, Mara runs into Bozzi in the parking garage, stuck because his car is blocked by a van and doesn't want to scratch his car. She tells him that she can try to get it out without leaving a scratch. Mara successfully gets the car out, but Carlo attacks the car and breaks the windshield with a wrench. Mara panics and backs up, crashing into another car. As Carlo nears the car, she tries to get unstuck from the other car. She eventually does, but crashes into a pole and breaks a taillight. After Carlo breaks the passenger window, Mara just decides to run from the car, back to Bozzi.

Bozzi, after going back to Padavo, phones Lukas and tells him to meet him there. Lukas leaves Mara at his family's house and goes to Padavo. While Bozzi is taking a bath, Carlo comes in and strangles him with the hose attachment. Lukas arrives in Padavo, only to hear about the murder from one of the locals. Realizing that the murders are about something that happened there, Lukas investigates Bozzi's house. He finds out from Bozzi's family about the Jewish family he betrayed and travels back to Rome.

After the maid leaves, Carlo targets Mara at the house. He calls her, but she asks how he got the number. Carlo sneaks into the house and tries to strangle Mara, but Ferrante, who had been ordered by Lukas to come retrieve her, saves Mara. Ferrante, then takes Mara to the judge's quarters, where Lukas went. On the way into the office, Carlo takes down Ferrante and captures Mara.

Lukas goes to the Judge's office where he finds a picture of his family, the same one Carlo left at Messori's door. The Judge appears behind Lukas and holds him at gunpoint. He explains that he was the one who sent the threatening phone calls to Bozzi, because he wanted them to feel guilty. He explains his role and asked if Lukas was going to turn him into the police. But before Lukas could answer, Carlo appears with Mara in his grasp. Carlo explained his warped mindset, claiming that they could kill Lukas and Mara and pin the murders on Ferrante. Coming to the realization that Carlo's too far gone and he is somewhat responsible for Carlo's downfall, the judge shoots his son and commits suicide.
